Community Care Service Program
(CCSP)
CCSP offers community-based services to people in need of in-home services to remain in the community. An individual must meet Medicaid Long-term Care Medical and financial guidelines. If under the age of 65, one must have a disability determination through the Social Security Administration or DFCS.
There is no fee for those individuals who qualify for SSI Medicaid. If you are above the SSI limit, you may be asked to pay for a portion of your services. A list of service options and the cost for these services will be provided to you before services begin.
Home and Community-Based Services
(HCBS)
The Area Agency on Aging has some funding to assist people over 60 with minimal services. This program is meant to give those in greatest need support to help them remain independent. The participant may be required to pay for a portion of their services based on income.
Due to limited funding, many counties have a waiting list for these services. The Area Agency on aging is mandated to serve people in the greatest need first. After a telephone assessment has been completed, a counselor will discuss the individuals’ risk level compared to others on the waiting list.
Money Follows the Person
(MFP)
This program assists people currently in the nursing home under Medicaid who may want to transition back into the community. The program provides funds that can help the person re-established back in the community. It also gives other types of support for one year after discharge.
